North East Lincolnshire Clinical Commissioning Group (CCG) and Services4Me have launched a new Dementia portal named, MyLife.

The aim of MyLife is to update the people of North East Lincolnshire (NEL) about information on developments, local services and plans to work towards improving standards of care for people with dementia and their carers.

The portal is still being built upon and will support continual improvements to the care people receive in North East Lincolnshire who recognise that there needs to be more public awareness and a commitment to responding more effectively to dementia.

The key information on the website will be:

• Reducing the risks – Information on the lifestyle changes that people can make and which may reduce their risk of developing dementia in later life;
• General information about dementia – Including the signs and symptoms of the conditions;
• Information for people who have had a diagnosis of dementia – Practical hints, tips and links to resources and services that can ensure people are able to live well with their condition for as long as possible;
• Information for supporting someone with a dementia diagnosis – Practical information on taking care of themselves and effectively supporting a person with dementia;
• Information for Social Workers and Health Practitioners –about dementia diagnoses, the local pathways for NEL and services that can be accessed for people with a dementia diagnosis and their support.
• Information about becoming dementia friendly in  North east Lincolnshire To help  people to stay independent for longer, and to actively reduce any admissions to hospital and care homes

Dr Karin Severin, Clinical Lead, North East Lincolnshire CCG said: “We wanted to bring this advanced idea to the people of NEL to bring together in one place essential advice and resources for people with the condition, and their carers. It is important to work in an integrated way to transform health and social care for people with dementia.

“We recognise the need to consult local people to ensure plans suit the needs of people with dementia, their carers and families and we plan to check out your views on a regular basis the portal will be another route to reach a lot of people.

“Despite the achievements we have made so far we know that change must go beyond health and social care, and further into our everyday lives.

“We have already made and sustained many improvements in recent years but there is still more to do and we must continue to shape our work and that of local providers and organisations and set out the plans to support people to live well with dementia in NEL.”

“We have been working closely in partnership with the CCG to develop the new Dementia portal and are delighted to add to this directory the new MyLife Dementia Portal. Which along with many other services, is enhancing this valuable online resource to give the public access to Advice, Information and Signposting to the local Health & Social Care economy.”
